• Payment is due when services are rendered. To focus on our patients’ needs, customer service, and minimizing office costs, we do not bill

• We accept debit cards, credit cards (Visa, Mastercard, Discover, American Express), checks, and cash. All cards must be signed by the owner of the card. Please note: There is a 2% fee on credit cards.

• We accept Care Credit. It takes just 5 minutes to complete an application and will allow you to split your payment into 6 monthly installments. You can apply online at www.CareCredit.com.

  • You must provide your ID with your Care Credit card. If the card is not in your name, you MUST be added as an authorized user to the account, as well as providing your ID.
